Medical Alert

Parents should notify the office and classroom teacher when they enroll their student regarding specific medical conditions which their student may have. This will enable the staff to be better prepared in the event of an emergency.

Upon enrollment, parents must complete an Annual  Health Status form indicating any specific medical conditions that their student might have. This will enable the staff to be better prepared in the event of an emergency.
